• Nurse Practitioner/Physician's Assistant

    Provide a scope of primary care services commiserate with experience in coordination with the collaborative partner, including but not limited to diagnosis, treatment, care coordination, preventive care and health maintenance to patients (2 years of age and above) in both a direct-pay patient setting and in support of a volunteer-based clinic model.  The provider will maintain an active collaborative agreement with the collaborative partner, including chart sign-off as mandated by state law.  The provider will also serve as leader to medical support staff as well as a resource to our volunteer providers and volunteer support staff.  The position will work alongside the Director of Clinical Services to ensure quality of care and support of our overall mission. The provider must be available to staff and patients after hours, if needed for consultation.  We are seeking a part-time (1 day per week, preferably Thursday) nurse practitioner (FNP) or physician’s assistant (PA). The provider will need to have a current licensure in good standing and be able to obtain licensure in Indiana. Applicants must have well-developed writing, interpersonal, and communication skills, as well as be able to use Electronic Medical Records for all charting. The position reports to the Executive Director.  
    Essential duties and responsibilities include the following (other assignments, projects, and duties may be required):

    • Provides continuing, comprehensive health maintenance and acute medical care to individuals and families--to include preventive medicine, behavioral sciences and community health.
    • Obtain health history and perform physical examinations.
    • Diagnose and direct counseling for each patient on a plan for treatment.
    • Prescribe medication in accordance with Indiana statute and professional practice guidelines.
    • Fluent with current standards of care in family medicine, including gynecology, pediatrics, geriatrics, and basic psychiatry.
    • Perform minor surgical procedures commiserate with training and licensure as well as collaborative agreement and Medical Director relationship.
    • Determines need for consultation and assists in medical care treatment provided at the direction of other specialists.
    • Arrange referral for patients based on need in coordination with the Clinical Services Manager.
    • Review incoming reports (e.g. lab, x-ray, EKG) sign, date and arrange follow-up in a timely manner.
    • Instructs, in conjunction with the Medical Director, other health care providers and non-medical personnel in a variety of health related topics, including first-aid measures and health care maintenance.
    • May participate in the instruction of students who participate in a clinical rotation at Hope Healthcare Services.
    • Assist in staff outreach and off-site recruitment/fundraising efforts as necessary.
    • Comply with the Hope Healthcare Services Infection Control plan which includes personally utilizing universal precautions. Ensures compliance by support staff.
    • Participate and ensure compliance with best practice protocols within Hope Healthcare Services as well as all regulatory compliance measures at the federal, state, and local level.
    • Participate in CME, peer review, quality assurance, provider meetings, and other clinical meetings.
    • Assist in updating protocols and principles of practice, as requested.
    • Adhere to and assist with demographic acquisition needs of the clinic and its supporting organizations.
    Physical Requirements:
    The physical requirements described here are representative of those that must be met by an employee to successfully perform the essential functions of the job. While performing the duties of this job, the employee is regularly required to sit; use hands to manipulate objects, tools or controls; reach with hands and arms; and talk and hear. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and the ability to adjust focus.
    • Family Nurse Practitioner (FNP) or Physician’s Assistant (PA) required.
    • Graduation from a college or university accredited by the American Association of  Colleges of Nursing (AANC) with a Family Nurse Practitioner degree or graduation from a college or university accredited by the Accreditation and Review Commission of Physician Assistant Education (ARC-PA) with a physician’s assistant degree
    • Current and unencumbered medical license within the US with ability to transfer license to Indiana
    • Current DEA certificate required.
    • Insurability (malpractice)
    • Current Cardiopulmonary Resuscitation (CPR) Certificate  
    • Previous computer experience or training required with particular emphasis on EMR use
    • Five years+ private practice or clinic setting (preferred)
    • Experience in public health (preferred)
    • Experience in missions or theological training (preferred)
    • Successful candidate must submit to TB testing and any vaccinations recommended by OSHA guidelines.
